This looks weird, and really broken.
After some tests, I was able to reproduce it, as follows:
- use small game graphics (the problem does not occur with normal sized graphics)
- play the game to create a new score entry -> the problem occurs now as described
- entering the scores from the main menu -> everything is fine (also as described)
I will have to investigate and fix this bug, which I have never encountered so far (as I normally does not use small sized graphics for normal playing)...

That short flash of the playfield using small-sized graphics in your video pointed me into the right direction! 
Found and fixed that nasty bug!
Thanks a lot, ncrecc, for your detailed bug reports! Especially those short video sequences of "how exactly does this bug look like" are extremely helpful, and worth more than many words!
